How your personal data is collected and processed

Personal and health information is collected by completing a consultation form prior to your first appointment. This is to to rule out any contraindications to treatment and to determine the most appropriate and beneficial treatment. This information is also used at subsequent appointments to evaluate improvements and any need to change the course of treatment.

Completion of a consultation form is an insurance related requirement.

No personal information is stored digitally.

Once I have received consultation forms by email I print them off and delete the original email file.

Your personal information is never disclosed to third parties.

Information is kept securely in a filing cabinet and is destroyed three years after the end of the treatment period (in accordance with regulatory and legal requirements.)

Personal contact information (name, email, phone, address) is kept for the purpose of scheduling appointments and if necessary to provide additional therapeutic information following treatments.
